The information you provide below will be shared with all volunteers and staff who may care for your child this year. All families are required to register after their first visit for the 2023-2024 program year.

RE Program Participation Agreement
I/We, being the legal parent(s)/guardian(s) of the listed minors above, do hereby give permission to UUFCO for said child to participate in the RE Program and related fieldtrips during the current church year. I/We do hereby release and discharge UUFCO from any liability of claim or cause of action that I may now have, or may have in the future, as a result of any injuries received by the said child(ren) while s/he/they participates in any activities and/or fieldtrips. Furthermore, in the event of an accident, I understand that an attempt will be made to contact me at the phone numbers provided on the Registration Form. If the emergency contact, or I, cannot be reached in time, I hereby authorize the administration of emergency medical aid to my child(ren). I agree to accept financial responsibility for any and all emergency medical care so provided. I/We agree to accept responsibility for the supervision of our young people while they are using electronic devices. I/We understand that UUFCO is a public space and cannot be held responsible for photos or videos taken and used by non staff members outside of official UUFCO publication spaces.
I/We understand that UUFCO staff and RE volunteers are mandated reporters in the state of Oregon. They are required to cooperate with all community safety reporting practices.
I/We acknowledge that official UUFCO activities require that two unrelated adults be present at all times with our young people, and understand this is the recommended practice for all unofficial activities as well.
I/We acknowledge that UUFCO RE programs are community led and volunteer supported. Parents/caregivers are asked to support their children in the RE classroom when needed and may be asked to accompany their child/youth during activities and events at any time. I/We commit to honoring the UUFCO Relational Covenant and all UUFCO policies and procedures.
I/We acknowledge that UUFCO RE programs use the annually updated RE Handbook as a guide. I agree to review this information and connect with RE Director about any questions.
